Catering:
We will assume that everybody attending the conference will take lunch, but hope that you will also stay on for dinner and enjoy hearing Professor Divya Chari, our keynote speaker on how cutting edge technology is revolutionizing medicine. Please indicate your preference on the form.

Transport to the Conference:
We are looking into whether there would be demand for assistance with transport to and from Concord from a central location in Shrewsbury, such as St. Chad’s Terrace, leaving at 10.30am to return for 8pm after the evening lecture. Please indicate your preference on the form.

** Please note that we will only offer transport if there is sufficient demand.  This will be confirmed by email within a week of the conference. **

** Please note that this is a student-only event. However, parents or carers are welcome to tea/coffee on arrival, and are welcome to a guided tour of the campus. **

The information on this form will be held in strictest confidence and used only for the purpose of communicating with you about the conference, after which it will be destroyed.

If however, you would like to receive information about further medical outreach events, such as our medic monthly evening lectures please indicate this on the booking form.
